Donations

Thank you for your interest in Mid-State Chevrolet. Our Charitable Donation process has changed to expedite the approval process. Each Sponsorship or Organization must complete the Charitable Donation enrollment form to be considered as a donation candidate.

Please complete the attached form and return to the receptionists. This form will be given to the Customer Relations Department to begin the approval process. Please allow up to 30 days to complete the approval and notification process.
